AHI North Country Population Health Improvement Program (PHIP) Stakeholder Meeting

The Adirondack Health Insititute's North Country Population Health Improvement Program (NC PHIP) is pleased to announce the 2018 in-person PHIP Stakeholder Meeting. Please join us at the Golden Arrow Lakeside Resort, Lake Placid, NY for dynamic discussions around the NC PHIP's and Partners' achievements and proposed plans for 2019.

Presentations and discussions will include: demonstration of North Country interactive health status maps, ADK Wellness Connections - a new resource and referral network, T-21 and other regional tobacco reduction activities and stakeholder priorities. A detailed agenda will be shared in early December.

Registration will begin at 9:30. The meeting will take place from 10:00 a.m. - 12:00 Noon.
